This template helps you find out how being in the peer support group is affecting members. It is a key part of impact evaluation. By using this survey, you can see what members like about the group and what could be improved. This feedback is crucial for making the group better and for getting funds, grants, or donations.

Regular impact evaluations, like every 6 or 12 months, help ensure the group continues to meet members’ needs and stays effective. You can use the survey template on the following pages or change it to fit your group’s needs.
